5. Business License # (upload state document for retail sale)

6. Item you are requesting with details: Item Name / Qty / Color / Fragrance / Add-Ons (Picture or words)

Invoicing: We will send you the invoice with the discount already processed on the order total.  For those who are going to be returning monthly, your business will be listed under the Cause Related Marketing tab on the Shop page to place orders with discounted pricing.
